Registration Instructions:
1. Fill out camp registration forms completely. Each child must have his or her own registration form.
2. Make sure your child’s age corresponds to the required age of each camp. Please use your child's age as of July 1, 2008.
3. Mail, fax, or drop off your completed registration forms with your payment to:

4. Full payments must be received by May 21, 2008.
5. Medical Forms and waivers must be filled out before the first day of camp in order for your child to participate in camp activities.

Upon receipt of payment a confirmation packet will be mailed to you including a health history form as well as medical and liability releases for each camper. These forms must be completed and returned to us. Your camper must have a completed health history form to participate in our camp program.
